In preparation for your first visit to Orthopaedic Associates of Michigan, please fill out and bring the Financial Policy and Privacy Notice Acknowledgement 2011 Form, which was mailed to you when your appointment was made. You may also want to complete the OAM Limited Patient Authorization Form for the release of your Protected Health Information to family members or persons assisting with your medical care. Having these forms completed in advance will reduce your registration time. Once you arrive, you will meet our medical staff and your physician.
At each visit we will ask for your insurance card and driver’s license or other picture ID. Children under the age of 18 must have a parent with them during each office visit. Payment is due at the time of service unless other arrangements are approved in advance.
We have contractual agreements with the carriers listed on our Insurance Participation List. Your insurance carrier is the best resource for your policy benefits and authorization requirements. Any required authorization for services must be obtained prior to your appointment or payment may be denied. As a courtesy, we will file an insurance claim for you.
For your convenience, we accept Visa, MasterCard, and Discover. Should you have any questions regarding our fees or your insurance coverage, please call us in advance of your appointment.
